Thank you for registering for Summer Music and Performing Arts Camp. You must complete this form before camp begins in order for your child to attend.

I hereby give permission for my child(ren) to attend Summer Music and Performing Arts Camp at Scottsdale Bible Church, June 8-12, 2009. In the event of an emergency in which my child is in need of immediate hospitalization, medical attention or surgery, and after reasonable efforts have been made to contact me or my spouse and we cannot be located for the purpose of consenting thereto, consent for the emergency attention may be given to any person standing to loco parentis to my child pursuant of A.R. S. § 44-133. I understand that my own medical insurance will be the primary coverage and that the SBC Insurance will cover any excess within its established limits. It is understood that my child will obey all regulations and follow the instructions of the leaders.
