Appointments
We do not consider our Counseling Center to be a crisis center, but rather, a short-term biblical counseling facility. If this is an emergency, please dial 911 or Magellan of Arizona at 1.800.564.5465 or Empact at 602.254.9000.
For regular appointments, please call our office manager at 480.824.7239. Appointments are often two to three weeks out, but should there be a cancellation, we may be able to get you in sooner.
After completing the intake information and appropriate forms, you will have an appointment with an intake counselor who will make an assessment in order to make an appropriate referral to one of the following:
- Stephen Minister
- Lay counselor
- Intern or Practicum Student
- Volunteer licensed professional counselor
- Staff licensed professional counselor
- Outside professional counselor
- Support groups
Our short-term counseling model (8–10 weeks) applies to a lay, pastoral or professional counselor who resides in our facility. We regret that we are unable to promise a specific counselor.
Forms
- Open the appropriate set of forms.
- Print them on your printer.
- Fill them out and sign, and then bring them into your scheduled appointment with you.
Couples Counseling
If you are seeking couples’ counseling (relationship, marital, etc.), please print out two intake forms: one for you and your partner.
Child and Adolescent Counseling
If you are seeking counseling for a minor, each family member needs to fill out their own form (if a family member under 18 will be attending, the appropriate intake form for their age needs to be filled out by parent or legal guardian, and the minor needs to fill out the Child and Adolescent form). We require parents to be actively involved in the counseling.

SBC Biblical Counseling Center Cancellation Policy
The Counseling Ministry at Scottsdale Bible Church is in high demand. We are grateful for the privilege to serve Him and you through this ministry. We strive to provide the best possible service and availability to all of our counselees.
Our policy is to reschedule missed appointments no sooner than two weeks after the missed appointment unless rescheduled or cancelled at least 24 hours in advance. If a scheduled appointment is missed or cancelled without 24-hour notice for a second time, we can not assure you that the appointment will be able to be rescheduled. Should you desire, we will place you on a waiting list and call when it is feasible for us to reschedule.
Please help us to serve you better by keeping your scheduled appointment. If you are unable to keep your appointment, please call 480.824.7239 as early as possible to reschedule or cancel.
